    A site supervisor, after quickly viewing the freshly staked locations for the guy tower's base and anchors, provided permission to his drilling contractor to proceed with the installation of the foundations. During an as-built survey, it was discovered that one of the outer anchors was located inside a utility easement. It was later found that a disgruntled employee of the general contractor had removed the original stakes, rotating all of them 12 feet counterclockwise. Had the site supervisor carefully reviewed the site plan, the error should have been noticed and a lot of finger pointing and additional expense could have been avoided.
